I´ve seen a lot of item questions in R/S/E Discussion, so I put all of them togheter. I´ll also post my opinions on them.

Battle Items:
Berries
*Recover Berries*

Aguav berry: Restores 1/8 of the holding pokemon's HP if it falls bellow half. Confuses if the holding Pokémon isn´tCalm, Gentle, Sassy, Careful Nature.

Sitrus berry: Restores 30 HP of the holding Pokémon's Health Point if it falls bellow half.

Wiky berry: Restores 1/8 of the holding pokemon's HP if it falls bellow half. Confuses if the holding Pokémon isn´t Modest, Mild, Quiet, Rash Nature.

Personal Opinion (PO): Do not use them. They are a waste of a slot. There are many hold items better than those, you´ll see when you finish reading.

Cheri berry: Cures paralysis.
PO: Not highly recomended, but you can try...
Location: Rotues 103, 104 / Flower Shop / Man in Lilycove city / Agate Village / Pick Up (FR/LG) / Pokémon Jump Mini-Game (Score above 5000 points)

Aspear berry: Cures freeze.
PO: Really not recomended
Location: Rotues 120, 121 / Flower Shop / Agate Village / Berry Forest / Pick Up (FR/LG) / Pokémon Jump Mini-game (Score above 5000 points)

Chesto berry: Cures sleep.
PO: Best use are with Curse+Rest Pokémon, like Snorlax.

Lum berry: Cures all status problems.
PO: Highly recomended, Syncronyze Pokémon greatly uses this in a switch. Also most fast sweper with it is good, you won´t get the attack halved with burns or the speed quartered with paralisis.

Pecha berry: Cures poison.
PO: If you fear toxic, the you can try this one, but Lum berry is many times better.

Persim berry: Cures confusion.
PO: Good to use in double battles, you use swagger in yourself, then recovers confusion!
Location: Rotues 104, 114, 121, 123 (Berry Master´s wife) / Flower Shop / Agate Village / Berry Forest / Pick Up (FR/LG) / Pokémon Jump Mini Game (Score above 5000 points)

Rawst berry: Cures burn.
PO: Like most of them, Lum is better.
Location: Rotues 112, 121 / Flower Shop / Agate Village / Berry Forest / Pick Up (FR/LG) / Pokémon Jump Mini Game (Score above 5000 points)

*Stat Boosting Berries*

Apicot berry: Raises Special Defense in one point (50%) when your HP is lower than 1/4.
PO: Isn´t very useful, holding Pokémon´s HP is already low so the defense won´t matter too much, so HP will rech zero in 99% of the time. Another option would be passing this boost to other pokemon via Baton Pass, but it´s not very recomended.
Location: Pokémon Colo/XD. Buy with 15000 Poké Cuppons

Ganlon berry: Raises Defense in one point (50%) when holding Pokémon´s HP is lower than 1/4.
PO: The same as the Apicot berry.
Location: Pokémon Colo/XD. Buy with 15000 Poké Cuppons

Liechi berry: Raises Attack in one point (50%) when holding Pokémon´s HP is lower than 1/4.
PO: This is more useful than the above berries, but it´s still not highly recomended, good users are fast Pokémon with high attack or with substitute. It can also be used with Baton pass, with a good result.
Location: Mirage Island

Petaya berry: Raises Special Attack in one point (50%) when holding Pokémon´s HP is lower than 1/4.
PO: Same than Liechi Berry. Plus: Sceptile is maybe the best pokémon to use this berry, specially the subseeding set (substitute + leech seed), in the low HP Grass moves deals more damage (x1.5) and with the Petaya even more (x1.5 again) turning Leaf Blade´s base damage 253.125 :eek: without critical hit.
Location: Pokémon Colo/XD. Buy with 15000 Poké Cuppons

Salac berry: Raises Speed in one point (50%) when holding Pokémon´s HP is lower than 1/4.
PO: The best choice for Subreversals (Substitute+Reversal or Flail), or Endure reversals. Good pokemons to use Salace berry are Dodrio, Heracross, Blaziken.
Location: Pokémon Colo/XD. Buy with 15000 Poké Cuppons

Starf berry: Sharply raises (x2 or +100%) a random stat when holding Pokémon´s HP is lower than 1/4.
PO: I don´t like Luck based items, imagine how good it´s when you double the Attack of one Blissey?
Location: Win the 7 Gold Symbols and talk to Scott

Lansat Berry: Raises Critical hit ratio when holding Pokémon´s HP is lower than 1/4. (2 CH boosts)
PO: Fun to be used, but even with the boost, you won´t get a great advantage.
No CH boosts: 6.5%
1 CH boost: 12.5%
2 CH boosts: 25%
3 CH boosts: 50%

These boosts can be from a move with a high CH ratio, Focus Energy, and/or Scope Lens/Lansat Berry.
Location: Get all 7 Silver Symbols and tlk to Scott

Beware of common Hazers or Roarers when using those Berries ;)

Charcoal: +10% fire moves.

Black belt: +10% fighting moves.
Location: Rotue 115 (Steal after 5th battle against BlackBelt Nob (IIRC Machamp holds it))

Dragon Fang: +10% dragon moves.
Location: Meteor Falls (Steal after 5th battle against Dragon Tamer Nicolas)

Black glasses: +10% dark moves.
Location: Rotue 116

Hard stone: +10% rock moves.
Location: Wild Aron/Lairon

Metal Coat: +10% steel moves.
Location: Wild Magnemite/Magneton

Miracle Seed: +10% grass moves.
Location: Petalburg Woods (Talk to the girl in the north eastern most part of the woods (HM Cut needed))

Mystic Water: +10% water moves.
Location: Weather Institute (attached to Castform when you recive)

Sea Incense: +5% water moves.
Location: Mt. Pyre, 4th floor

Silver Powder: +10% bug moves.
Location: Rotue 120 (Steal after 5th battle against Bug Maniac Brandon (Masquerain holds it))

Magnet: +10% electric moves.
Location: Trick House (pass trought 6th maze)

Soft sand: +10% ground moves.
Location: Wild Trapinch

Nevermeltice: +10% ice moves.
Location: Shoal Cave (Blue area, where you get Snorunt)

Poison barb: +10% poison moves.
Location: Wild Roselia/Cacnea

Spell tag: +10% ghost moves.
Location: Wild Duskull/Duscops/Shuppet/Bannette

Sharp beak: +10% flying moves.
Location: Wild Doduo/Dodrio

Twisted spoon: +10% psychic moves.

PO: NEVER use this section of items, they are TERRIBLE! Especially Sea incense.

Specific pokemon boosting stats/abilities items:

DeepSeaScale: doubles clamperl's sp def.

DeepSeaTooth: doubles clamperl's sp attack.
PO on both above: I need say something here?
Location: Give Capt. Stern the Scanner and he´ll give you one of those.

Light Ball: Doubles Pikachu's Special Attack.
PO: You can try, but there are a LOT of better electric Pokémon.
Location: Wild Pikachu

Lucky Punch: Gives Chansey a higher critical hit ratio. (look Lansat Berry quote)
PO: LOL!
Location: Pokémon XD, Snag Chansey and it´ll hold it.

Metal powder: doubles ditto's defense.
PO: No one uses Ditto for battling, so I have nothing to say here.
Location: Wild Ditto.

Soul dew: +1 point (x1.5) in Latios´ S.Defense and Latias´ S.attack.
PO: If you have it, USE IT!
Location: Eon Ticket Latias/Latios.

Stick: Gives Farfetch´d a higher critical hit ratio. (look Lansat Berry quote)
PO: Few people uses Farfetch´d and the raise in CH ratio isn´t so great.
Location: Wild Farfetch´d/ XD Farfetch´d

Thick Club: Doubles Cubone's and Marowak's attack.
PO: When you see Marowak, you see Thick Club, so use it when you use Marowak.
Location: Wild Cubone/ XD Marowak

Other items:

Leftovers: Restores 1/16 of your Pokéemon's HP after every turn.
PO: THIS IS THE ITEM. But please don´t waste it in Pokémon with poor defences.
Location:
R/S: Battle Tower, beating it 6 times in a row will have the chance to get it after beating 7 trainers.
Emerald: Battle Shop, Far right Kiosk, buy with 48 Battle Points

Quick Claw: Has 10% chance to move first in battle.
PO: High luck based item, you can try, but don´t expect too much of this thing.
Location: Rustboro City, Talk to the teacher in the Pokémon Trainer School.
Emerald: Battle Shop, Far right Kiosk, buy with 48 Battle Points

Brightpowder: Increases evasion by one point (this is the same than lower opponent´s Acc. Attack by 9.22%)
PO: Another Luck based item, if you have nothing to give for a tank or wall, use it, if you already used leftovers in another Pokémon.

R/S: Battle Tower, beating it 6 times in a row will have the chance to get it after beating 7 trainers.
Emerald: Battle Shop, Far right Kiosk, buy with 48 Battle Points

Lax Incense: Lowers opponent acc. by 5% (subtract from attack´s acc.).
PO: A powerless Brightpowder, but if you have it, use it to BREED Wobbufet, you´ll hatch a future Uber Pokémon. Useless for battles.
Location: Mt. Pyre, 5th floor.

Mental Herb: Instantly Cures infatuation.
PO: Another "I´ll never see this" item.
Location: Foretree Cit, return boy´s Wingull back to him.
Emerald: Battle Shop, Far right Kiosk, buy with 48 Battle Points

King´s Rock: Raises the chances on dealing a Flinch by 12%.
PO: You MUST atack first to make this item usable, also check if the attack uses King´s Rock here.
Location: Wild Hariyama/ Boy standing near Steven´s house.
Emerald: Battle Shop, Far right Kiosk, buy with 64 Battle Points

Scope lens: Increases Critical Hit ratio. (look Lansat Berry quote)
PO: Even with the Boost in CH ratio this won´t be too useful, so it´s not highly recomended.
Location:
R/S: Battle Tower, beating it 6 times in a row will have the chance to get it after beating 7 trainers.
Emerald: Battle Shop, Far right Kiosk, buy with 64 Battle Points

Shell bell: Recovers 1/8 of HP dealt in the opponent. (Ex. if you took 80 HP of someone, you´ll recover 10HP)
PO: Just use in good swepers, like Dragon Dance Salamence.
Location: Shoal Cave, exchange 4 Shoal Shells and 4 Shoal Sands to the oldman near entrance.

White herb: When it gets any stats lowered it will return to normal instantly.
PO: Good to use in Pokémon that uses Overheat or the passes Curse (When you use Curse then Baton Pass in the White herb Pokémon, the Speed will return to normal)
Location: Rotue 104, Right next to the Flower Shop
Emerald: Buy in Battle Shop in Frontier with 48 Battle Points, or steal Flannery´s White Herbs (Beware of Arcanine´s Intimidate, make sure you have Recycle (Mr.Mime) or the Covet Pokémon isn´t affected by it.

Choice Band: Multiplies holding Pokémon´s Attack by 1.5 (or gives +50%), but you can´t change attack when this Pokémon is in the arena.
PO: Slaking, Metagross, Salamence..., will always have the chance to use this item, with the boost, Metagross and Salamence´s attack will reach at most 607, wich is an extreme advantage. Another thing you can do is trick, use it in a slow special sweper Pokémon and take it´s item!

R/S: Battle Tower, beating it 6 times in a row will have the chance to get it after beating 7 trainers.
Emerald: Battle Shop in Frontier, buy it with 64 Battle Points.

Macho Brace: Doubles Effort points recived in battle, but lowers speed.
PO: You can rarely see this in battles, the only use for it is lower opponent´s speed, so use the strategy wrote before, use trick to steal some berries or better items for that moment like leftovers. It´s not recomended, choice band+trick is better.
Location: Win 4 battles against Winstrate family.

Recover Items:

Antidote: Recovers from Poison.
PO: Don´t use it, the 50 Pokédollars you earn selling it is more useful.
Location: Marts

Awekening: Recovers from Sleep.
PO: Don´t use it, the 50 Pokédollars you earn selling it is more useful.
Location: Marts

Blue flute: Recovers from Sleep. (Infinite)
PO: Use it! With Blue flute you can awake infinite Pokémon, hundred times better than Awekening.
Location: Rotue 113, exchange 250 steps walking on volcanic ashes.

Burn heal: Recovers from Burn.
PO: Don´t use it, the 50 Pokédollars you earn selling it is more useful.
Location: Marts

Elixir: Recovers 10PP from all Pokémon moves.
PO: Maybe the only local you´re going to use this is in Battle Pyramid.
Location: Rotues 119 and 123

Energy Root: Recovers 200HP. Bitter.
Heal Powder: Recovers any paranormal status (Burn, Frozen, Poisoned, Paralyzed, Asleep, Confused, and IIRC Infatuated). Bitter.
Energy Powder: Recovers 50HP. Bitter.
Revival Herb: Recovers a fainted Pokémon. Bitter.
PO on above four: Just use those if you like Frustation more than Return, but I don´t reccomend, if you just walk with your pokémon, it´s hapiness will raise :/
Location: Lavaridge bitter shop.

Ether: Recovers 10PP from a selected move.
PO: Again, the only local you´re going to use this is in Battle Pyramid.
Location: Rotues 113 and 116 / Petalburg city / Petalburg Woods

Fresh water: Recovers 50HP.
PO: It´s useful when you´re starting your travel, and IIRC it´s cheaper than Super potion.
Location: Lilycove Dept. Store.

Full heal: Recovers any paranormal status (Burn, Frozen, Poisoned, Paralyzed, Asleep, Confused, and IIRC Infatuated).
PO: Use it! Use the money you earn selling Antidotes or Ice heals to buy this item, it´s really useful!
Location: Marts

Full Restore: Recovers all Pokémon´s HP and recovers paranormal status.
PO: it´s the best item in the journey, you can also get the money from Ice heal etc to buy this!
Locaiton: Ever Grand City, 3000 Pokédollars

Hyper Potion: Recovers 200HP.
PO: Pokémon center.... :rolleyes:
Location: Marts

Ice Heal: Recovers from Frozen.
PO: "items that you´ll never use"
Location: Lilycove Dept. Store

Lava Cookie: Recovers any paranormal status (Burn, Frozen, Poisoned, Paralyzed, Asleep, Confused, and IIRC Infatuated).
PO: It´s cheaper than Full heal, but you´ll waste time talking to that oldwoman to buy 10 of those...
Location: Mt. Chimney

Lemonade: Recovers 80 HP.
PO: Same for Fresh Water, it´s useful when you´re starting your Journey, but Hyper potion is better...

Max Elixir: Recovers all PP from all Pokémon´s moves.
Location: Victory road, Aqua/Magma Hideout
Max Ether: Recovers 10PP from a selected move.
Location: Rusturf Tunnel / Rotue 113 / Mt.Pyre
PO on above two: I sell them. Rarely you´re going to use those items.

Max Potion: Recovers all HP.
PO: Full restore is better, but this is a variant, if you don´t have a paranormal status, use this.

Max Revive: Revives a Pokémon to it´s Max HP.
PO: Sell them, I use more Revives than Max Revives.
Location: Pick Up in Emerald / Petalburg City (use Surf)

MooMoo Milk: Recovers 100HP
PO: I love this item ^^ it got a creative name, but it´s effect... Full restore!
Location: Wild Miltanks (Emerald Safari zone)

Paralyz Heal: Recovers from Paralysis.
PO: Like I wrote before, sell it.
Location: Marts

Potion: Recovers 20HP.
PO: The most common Trainer card in TCG, is also the most common item in the start.
Location: Marts

Red Flute: Recovers from attraction. (infinite)
PO: Like Blue flute, Use it. I always keep the Flutes in the start of my item list. They´re so useful!
Location: Rotue 113 ; 500 steps on volcanic ashes.

Revive: Recovers a fainted Pokémon to half of it´s HP.
PO: Sell Max Revive, buy Revive ;)
Location: Marts / Emerald Pick Up

Sacred Ash: Recovers all fainted Pokémon in your party.
PO: An item you´re just going to use in Battle Pyramid, it´s the best item there.
Location: Mystic Ticket, Ho-oh will hold it. Or wild in Battle Pyramid.

Soda Pop: Recovers 60HP.
PO: Meh...

Super Potion: Recovers 50HP.
PO: Fresh water :rolleyes:

Yellow Flute: Recovers from Confusion. (infinite)
PO: Repeating, Flutes are the best items for a journey.

Blue shard: Trade for Water Stone in R/S/E at the house near Mosdeep city. Wild Clamperl Hold them sometimes.
PO: They´re rare, but good!
Location: Wild Clamperl or wild underwater (check darker spots, or use Itemfinder)

Calcium: Gives +10 EV in S.attack. (if the pokémon has more than 100 EV in S.attack, you can´t use this item)
Carbos: Gives +10 EV in Speed. If the pokémon has more than 100 EV in Speed, you can´t use this item.
HP UP: Gives +10 EV in HP. If the pokémon has more than 100 EV in HP, you can´t use this item.
Protein: Gives +10 EV in Attack. If the pokémon has more than 100 EV in Attack, you can´t use this item.
Zinc: Gives +10 EV in S.defense. If the pokémon has more than 100 EV in S.defense, you can´t use this item.
Iron: Gives +10 EV in Defense. If the pokémon has more than 100 EV in Defense, you can´t use this item.

Escape rope: Teletransports you to the last PC visited if you are in a cave/dungeon.
PO: I like to use them while catching Dittos.

Fire stone: Use it to evolve Vulpix, Growlithe or Eevee.
PO: Duh...
Location: Rotue 124, Exchange Red Shards / Fiery Path (Strenght) / Buyable in FR/LG

Flufy tail: Use it to escape a wild Pokémon battle.
PO: Useful in Pyramid, but... I never waste money with this...

Green Shard: Trade for Leaf Stone in R/S/E at the house near Mosdeep city. Wild Relicanth hold them sometimes.
PO: Let´s repeat, They´re rare, but good!
Location: Wild Relicanth or darker spots (or white rocks) while diving

Heart Scale: Trade for move re-learner in R/S/E.
PO: "Most Useful Items From The Game".
Location: Wild Luvdiscs / Lilycove beach (use Itemfinder)

Leaf Stone: Use it to evolve Gloom, Weepinbell, Exeggute or Nuzleaf.
PO: Another Duh item!
Location: Exchange Green Shards / Buyable in FR/LG

Max Repel: Repels wild Pokémon with the level lower than your leading Pokémon´s level for 250 Steps.
PO: Sell it, you get an advantage using Super repel.
Location: Marts / Lilicove Dept Store.

Moon Stone: Use it to evolve Nidorina, Nidorino, Clefary, Jigglypuff or Skitty.
PO: OMG, so many Duh items!
Location: Meteor Falls / Wild Lunatone.

Nugget: Sells high.

Easiest locations: Steal from Rick Boy´s or Lady´s Linoone / Emerald Pick Up

Pearl: Sells low.

Pokédool: Use it to escape a wild Pokémon battle.

Location: Buy Flyffy Tail if you really want to escape... you can´t get it in R/S/E

PP MAX: Raise PP of a Pokémon´s move to the highest possible.
PO: Best Item from the game, also some people don´t know about it´s existance in the Magma Hideut, that´s why I added it.
Location: Trick house in R/S/E, Magma hideout in Emerald.

PP UP: Raise PP usage of a Pokémon´s move.
PO: PP MAX are better (of course), but PP Ups are great too.
Location: Meteor Falls / Victory Road / Rotues 104 and 109 / Lilycove Loto (Match last 2 numbers)

Rare candy: Raises a Pokémon´s level by one.
PO: The word Duh is getting repetitive here...
Location: Rotues 106, 110, 114, 119, 120, 123, 132 / Trick House / Pick Up

Red Shard: Trade for Fire Stone in R/S/E at the house near Mosdeep city.
PO: Rare but good!!
Location: Wild Corsolas

Repel: Repels wild Pokémon with the level lower than your leading Pokémon´s level for 100 Steps.
PO: Super repel is better, when you find one of those, sell it :) .. well toy can try those to catch Latias/Latios
Location: Marts

Yes, they really are that bad. 10% isn´t a great advantage and it will boost just one of your attacks (repeating type in moveset (flametrower and fire blast in the same Pokémon) is bad).

Ditto is very slow and high defense with low total HP is worse than low defense and high HP (Best example on that are Schuckle (high defense low total HP) and Blissey (Low defense and High HP)), so any good placed attack 1HKOs it before it thinks in transform.

What about Explosion, what can a really strong move do?

Pretty good, but the moves have to be strong enough to really make a difference.

I´ll take the example of Alakazam (let´s consider it has 340 in S.attack), if you use Psychic in one Pokémon with 269 S.defense and 310 HP it will take more or less 43% of the HP (staying with 168 HP) now if the same Alakazam uses Psychic holding Twistedspoon in the same Pokémon with 269 S.defense and 310 HP it will take 48% of the HP, (staying with 152 HP).

Just 16 HP diference, wich isn´t so great.

That Pokemon seems to have quite a bit of Special Defense. For important and strong moves, the Pokemon gets a good advantage. My Metagross actually ko'ed a Groudon in one hit with Meteor Mash and it was holding a Metal Coat.

Anyway, you might want to note that some any items that are related have a difference according to Attack and Defense.

Attack: Shell Bell
Defense: Leftovers

Attack: King's Rock
Defense: Brightpowder

I could really use some Berry Juice right now.

- what is it's chance of preventing your pokemon from fainting?

This isn't a in-battle item, but I'd still like to know; where do you get a smoke ball? I have a guide that says from trick house and wild koffing, but the serebii pokedex says koffing never have an item. Which is it? Can I get it from koffing, or do I have to beat trick house?
