It's impossible, even with a cheating device.

One form appears on R/S, FR, LG and E and as soon as you put it on a new game, the form changes so there is no way at all to put all the forms in one game.

Only 1 form is made per version. RS, FR, LG, and Emerald all have their own version that's on the data for their respective games.
