Pokemon FR/LG has no in-game clock -- at all. All in-game events are triggered by steps. (You can demonstrate this by riding in circles around Daisy's house -- after enough steps, she'll give you another haircut. Likewise, the items on Treasure Beach "regenerate" after enough steps, though I believe you have to switch routes first.)

Because of this, how does Pokerus work? I have it on my Sapphire, and I'm considering transferring some to FR/LG (once I get access to another Game Boy Advance, which won't be for a while). How long would the spreading effect last? Would it be based on in-game time, or based on steps, or is it permanent?

i thought that it didn't spread on FR/LG, cos it never did on mine,i think thepokemon keeps it until it goes back to a time based game, but i'm not sure

It can be permanent but only if your lucky, it's permanent on my Blaziken, it has the Pokevus for 3 months now.

Pokérus doesn't spread on FR/LG. Your Pokémon (traded in) with Pokérus still experience the EV benefits of it, however. (I'm effort training a Pokérussed Huntail in the Lavender Tower as we speak.)

Now, not having a clock isn't what makes Pokérus not spread; it just makes it so that it never goes away. The fact that Pokérus doesn't spread on FR/LG was a balancing decision on the part of Game Freak. They felt it would be cheap to have Pokérus that spreads AND lasts forever, so they made it unable to spread to keep things fair.
