...how does the game decide where you go?

In Colosseum, it's just whichever PC you last saved at.

In the GB games, it's whichever Pokémon center you last healed at.

What is it in XD?

I always end up in the HQ lab whatever I white out.

I think it's still the last place you've healed your Pokemon. The lab is where you start the game and where you heal your Pokemon.

While going through the Cipher lab I healed my Pokémon just inside the door next to the PC, after using that PC to switch my Pokémon around, however I kept ending up back at the HQ lab whenever I whited out.

Obviously it isn't just the last place you healed.

It takes you back inside your room at Pokémon Lab HQ, everytime you white-out in battle.

Not true, all through the Cipher Lab, whenever I whited out I ended up in the HQ lab.
Then when I lost to Lovrina I ended up in the entrance of the Cipher Lab...

Maybe it depends on who you lose to.

When I first whited out, I was in the lab. I'd guess that's where you go.

When you white out, you take to the place that you last saved. (If you snag'd, the snag'd POKéMON is lost!)

No, that is not true either. It is different for different locations. In most battles, it is the HQ Lab, some it does not matter if you win or lose, you just continue either way, and some (like w/ Greevil) it takes you back to a recent PC. But any way, you still lose the shadow. Hope that helps.
